Aged Care Facilities
Residential Medication Management Reviews (RMMR) have been designed to enhance the quality use of medicines for residents in an approved Australian Government Funded Aged Care Facilities, by assisting consumers and their carers to better manage their medicines.
Once the residents GP has issued a referral to one of our Consultant Pharmacists, we will conduct a RMMR in collaboration with the GP and other health care professionals of the Aged Care Facility.
We aim to identify, resolve and prevent medication related problems based on the residents’ medical history, whereby a comprehensive assessment is undertaken and reported back to the GP.
Other Aged Care Services We Provide
Quality Use Medicines (QUM) promote the safe and effective use of medications and correct management of them within ages care facilities. These can include, but are not limited to, participating in:
- Medication Advisory Committees
- In service clinical education - disease state management & medication management
- Advice on policies and procedures regarding medication use and administration
- Comprehensive data and drug usage reports (psychotropic)
