Home Medicines Review (HMR)

A HMR is a government funded initiative that allows people in the community to have their medications reviewed by a qualified consultant pharmacist.

It is designed to assist and maximize the benefits of one’s medication and reduce or prevent medication-related problems.

Patients taking multiple medications, have chronic illnesses, are recently discharged from hospital or experience difficulty managing their medications are eligible for a HMR following a General Practitioners referral.

Following a referral from the GP, an appointment will take place with one of our qualified Consultant Pharmacists whom have been accredited with the Australian Association of Consultant Pharmacy to conduct a review of your medications at either your residence or a location suitable for you.
